I have tested in both Firefox and Chrome and I get the same 404 Sorry the link may be broken or the page no longer exists.
I have looked in the Chrome Network page and all requests are http 200 successful.
I have looked in Chrome Console and there are no error messages. So Facebook is processing the request for the page.
The site where the app was disabled is www.911tap.org
All custom social media icons have been replaced with the official branded icons from each social media venue so that the brands of each are honored.

There is no form available at developers.facebook.com/appeal for submitting an appeal. It is just an information page as follows:

So I do not see that there is any course of action I can take except for making an appeal here. Perhaps Facebook has rolled out a new AI platform that serves different pages to different users based on the context of their use of Facebook or based on the context of the content of their website which is linked via the App ID to Facebook.
