I have deleted the channel, the user map and the user, 3 or 4 times during my own investigation and have recreated it but with no luckalzander wrote: Everything looks right from what you describe. * Recreate the channel from scratch

Facebook is constantly updating their Graph API and new apps always use the latest version, currently 2.4. Your 'old' app would be using whatever version was latest when it was created. So, there's a possibility that the new API version has a change we're not aware of, though no other users have reported this and 2.4 has been the most recent for about 1.5 months.
Let us know how the above goes and sorry to make you redo things. It should be a pretty quick process though.
