Very strange. I created a new channel in your backend, with my user as the Channel Owner. Then I went to the front-end end and hit the login with Facebook button. I received the following two prompts. After saying yes to the prompts, I then went back in to the channel and selected a page. When I saved it, the test stream had valid data.
However, I did get it into a weird state with no permissions prompt if I deleted my user mapping, but had not saved the channel again.
Could you try the following steps in this
exact order to try to help narrow down/fix the problem?
1. Log out of the front-end of your site.
2. Go into facebook.com/settings for your user that is an admin of your app.
3. Click on the Apps tab on the left and then remove your application when the list is populated.
4. Go into your backend and go to JFBConnect > User Map.
5. Remove the user entry with that user for Facebook.
6. Delete the channel from your backend.
7. Go to your front-end and log-in with your Joomla username and password.
8. Hit the connect to Facebook button. At this point, you should only see one of the prompts above for posting to your page.
9. Once connected, log out of the front-end.
10. Create the channel in the backend.

Make sure to hit the Save button, even though you are encountering the manage_pages warning. If you don't hit Save, JFBConnect will not know that it needs to prompt the user on the next login attempt. *This is something that I have added as an issue to investigate for a new release.* Note: I don't know if you had a channel, but deleted it or if you just never hit the save button once you saw this warning.
12. Login with your Facebook user on the front-end.
13. At this point, you should see both of the prompts above and accept.
If you get to this point after following the steps exactly, and still don't see the manage pages prompt, please let me know
14. Go back into the channel, refresh it and then select from the available pages.
