The user's phone number and street address are not fields that Facebook returns. There's no way to get this information from Facebook due to privacy considerations.
As for the other fields, what component are you integrating with to import this information into (Community Builder, JomSocial, Kunena, etc)? In the field map drop-down of the Profiles area in JFBConnect, there should be settings for "Basic Info - Birthday" and "Basic Info - Current Country" or "Basic Info - Hometown Country". If you use those settings is nothing returning, or are you not seeing those values in the configuration options at all?
