Login with facebook redirect to error page and Object and Action not

Please, when my users visit my site and login in automatically with their facebook it pops out massage the facebook profile has been connected to my site and redirect user to error page (0 - Could not connect to MySQL.

I follow the open graph video installation guide and still having issue, cant see any update in the user timeline for both OBJECT and ACTION (using K2 items) hope bellow info could help
